A gateway to team-based care

The Gateway to Team-Based Care will be a provincial centre for collaborative health education, interdisciplinary research, and innovation in team-based primary care. It aims to build capacity for collaborative health education to advance and accelerate the delivery of patient-centered, team-based primary care. It will support learners from across the health disciplines to work and learn in a team environment through collaborative health education—building capacity of health professional graduates and a future workforce prepared to work in team-based care that will ultimately enhance patient and caregiver experience and improve the health of people in British Columbia.

In 2022/23, UBC Health began a comprehensive engagement strategy in partnership with nine health professional programs to develop the Gateway to Team-Based Care. The project is being led by an oversight committee that involves an unprecedented degree of collaboration. UBC Health has integrated patient partners and health professional students into the design process, developed new relationships with health sector partners to support the advancement of interprofessional practice education, and engaged leaders in the BC Government to establish the Gateway to Team-Based Care as a provincial resource that will build capacity for team-based primary care across the province.

Strategic Objectives

The Gateway to Team-Based Care advances the following UBC Health strategic objectives:

  • Education ecosystem in team-based care: Support the establishment of a team-based clinic on campus and connect existing campus and other clinics as learner-informed models for team-based education
  • Public and patient engagement: Expand engagement to advance diverse community priorities, enhance learner experiences, and align research activities
  • Coordinated responses: Coalesce experts and efforts to further collaborative health education and interdisciplinary research in response to pressing health challenges
  • Evidence for new approaches: Help align university and sector activities to establish evidence for new approaches to health
