12:00 - 1:30 PM (Online via Zoom)

Join us for an engaging online session on a province-wide evaluation initiative from the TBC@UBC Network (UBC Health), led by UBC’s Innovation Support Unit (ISU), to strengthen team-based primary care (TBC) across British Columbia.
Developed collaboratively with primary care partners and funded by the Ministry of Health, this initiative focuses on translating evaluation findings into practice, improving experiences for patients and providers, enhancing care quality, and supporting the long-term sustainability of the health system.

The BC Team-Based Primary Care Evaluation involves up to 30 primary care teams across a range of models and regions. Using a structured action research approach, the ISU collaborates with participating teams to collect data through three key tools: a patient experience survey, a team survey for providers and staff, and a clinic-level report. The evaluation focuses on five areas drawn from the TEAM Framework: patient experience, provider experience, team function, capacity and access, and patient attachment.

In this session, you will:

  • Learn about the ISU’s collaborative evaluation design and how it supports primary care teams in moving from evidence to action
  • Hear key insights and lessons from Round 1, including considerations for evaluating and implementing quality improvement in primary care clinics and health centres
  • Review emerging provincial-level findings and how they are being shared with both decision-makers and clinic teams

Following the presentation, join small group discussions to explore:

  • What enables or hinders quality improvement in primary care
  • Best practices for engaging patients in evaluation efforts
  • Opportunities for your clinic or team to participate in current or future phases of the evaluation

Whether you're a provider, clinic leader, patient partner, policymaker, or researcher, this session offers practical insights into the evolution of team-based primary care in BC, and how we can collectively move from evaluation to meaningful action.

More information about the Innovation Support Unit (ISU)

The Innovation Support Unit (ISU) is a small and nimble research team within the UBC Department of Family Practice that is dedicated to improving primary care and team-based care. We develop innovative tools, methods, and approaches to help teams explore new ways of working together and provide support to communities as they implement team-based care models across healthcare settings. We are the unique merging of an academic research unit, a think-tank, a consultancy, and a primary care change lab. We work closely with many partners and groups across British Columbia and Canada e.g. communities, Ministry of Health staff, patients, clinical staff, Divisions, and academics. Our work includes:
