TBC@UBC: Learnings and Reflections from the BC Team-Based Primary Care Evaluation

Join us for an engaging online session on an important province-wide evaluation initiative from the TBC@UBC Network (UBC Health), led by UBC’s Innovation Support Unit (ISU), that focuses on translating learnings into action.
Developed collaboratively with primary care partners and funded by the Ministry of Health, the BC Team-Based Primary Care Evaluation seeks to better understand the current positive benefits of the transition to team-based primary care (TBC). It focuses on five areas of the TEAM Framework: patient experience, provider experience, team function, capacity and access, and attachment.
The evaluation aims to engage up to 30 primary care teams across diverse models of care and regions in the province. Using an action research approach, the ISU works with participating teams to gather information using three tools (a patient experience survey, a team survey for providers and staff, and a clinic report) and then co-create actions for quality improvement from the evaluation learnings.
In this session, you will:
- Learn about the ISU’s action research approach and how it supports primary care teams to move from evidence to action
- Hear reflections and lessons learned from the evaluation so far, including key considerations for primary care evaluation and how to support teams in using data to inform quality improvement
- Learn about emerging provincial-level findings
Following the presentation, you will participate in small group discussions to explore:
- What factors enable quality improvement in primary care and what challenges exist
- Best practices for engaging patients in primary care evaluation
- How your team could participate in the evaluation
Whether you are a provider, clinic leader, patient partner, policymaker, or researcher, this session offers valuable insights into the evolution of team-based primary care in BC and how we can collectively move from evaluation to action.
