TBC@UBC: Impact of Teamwork in Primary Care through HDC Discover Measures

Dr. Cole Stanley, Medical Director, Health Data Coalition

Dr. Cole Stanley is a Vancouver-based family physician with nine years’ experience practicing in BC. He received his MD from the University of Toronto and completed his Family Medicine Residency at the Jewish General Hospital, McGill University. Dr. Stanley is a clinical instructor with the UBC Faculty of Medicine and a family physician with the Hope to Health Research and Innovation Clinic in Vancouver’s Downtown Eastside.

Cathy McGuinness, Manager, Community Engagement, Health Data Coalition

Cathy McGuinness is a member of the Clinical Services Team and brings over thirty years of experience in operations management, product development, business development and sales within the healthcare sector. As access to aggregate community data is a new concept, Cathy’s role is to support Divisions and Communities in understanding how to align priorities with community aggregate data, leverage Learning Programs with the use of community measures and drive change with quality improvement.

The Health Data Coalition (HDC) is a physician-led organization that supports a data network that provides clinical practice measures, as a service, for family physicians to better understand their practice processes, patient panel, and patient outcomes. HDC is a trusted, timely, and insightful data source as it aggregates health data across six different Electronic Medical Records (EMR) solutions across geographical communities and health authority regions in BC to empower care providers to recognize trends, identify strengths, and consider opportunities in their unique practices.

Our purpose is to enable data-informed patient care through practice improvement to support the quintuple aim to support our larger vision of a learning culture where primary care data is transformed into applied knowledge. The ultimate value of our efforts will be seen in improved outcomes for patients.

HDC’s Clinical Data Stewardship Committee (CDSC) is a unique working group of experienced physicians and members of our product development team. The CDSC focuses on impacts on quality of care for patients, and adding measures to HDC Discover that are relevant to common primary care problems. The CDSC also aims for HDC Discover measures embedded in QI initiatives, moving away from measures for measures sake.
