TBC@UBC: Impact of Teamwork in Primary Care through HDC Discover Measures

How can interprofessional primary care teams assess whether their efforts are making a meaningful difference during a primary care crisis?

This session will highlight three examples from across the province where team successes were recognized through clear, measurable impacts in the HDC Discover application. This type of feedback mechanism not only validates team efforts but also serves as a powerful motivator, reinforcing the connection between collaborative work and improved patient outcomes.

We’ll explore how real-time feedback supports teamwork, especially for developing teams, and why it is a critical component of continuous practice improvement.

The Health Data Coalition (HDC), a physician-led not-for-profit organization, provides clinical measures through its HDC Discover application. Built on a primary care data network, HDC Discover aggregates data from six Electronic Medical Record (EMR) systems across BC, offering a unique lens into practice patterns, patient panels, and health outcomes.

The TBC@UBC Network is excited to host this online event, showcasing how HDC Discover empowers teams to transform clinical data into actionable insights, fostering collaboration, and driving meaningful improvements in patient care.
